Snus causes gum damage — but white pouches are a research blind spot
A new European review of 26 studies confirms snus causes gum recession and inflammation. But nicotine pouches? Researchers found almost no long-term oral health data.
A new Swedish research review confirms what dentists have been seeing: snus causes gum recession and inflammation, especially where the pouch sits. But the fast-growing category of white nicotine pouches — the tobacco-free products now popular with young users — remains almost completely unstudied.
Researchers at the University of Gothenburg reviewed 26 European studies on Swedish snus and oral health. The findings are clear for traditional tobacco snus, but highlight a major knowledge gap for the newer nicotine pouch market.
What the research shows about snus
Traditional snus is linked to gum recession, particularly at the placement site in the mouth. Older studies also show increased gum inflammation among users.
The damage isn't just mechanical pressure from the pouch. "We see that snus affects the gums locally," says Mats Jontell, professor emeritus in oral medicine and pathology at Sahlgrenska Academy. "There also appears to be an inflammatory reaction that leads to bone resorption, which results in gum recession over time."
The good news: the research found no clear evidence that snus causes destructive periodontitis — the type of disease where teeth progressively lose their attachment to the jawbone.
Portion snus shows lower risk than loose snus for gum recession. Newer portion products may reduce the risk further, according to the research.
Cavity link unclear
Results on cavities are contradictory. Some studies show more filled or decayed teeth among traditional snus users; others see no difference. Bottom line: researchers can't confirm that traditional snus directly increases cavity risk.
Pregnancy signal
One of the larger studies in the review found that children born to women who used traditional snus during pregnancy showed a tendency toward increased risk of cleft palate compared to non-users. The risk was not elevated among women who quit snus before prenatal care enrollment.
"Even though this is a risk increase and not proof of causation, it's an important signal from a public health perspective," says Maria Bankvall, university lecturer in dentistry at Jönköping University. "Snus use during pregnancy cannot be considered risk-free, especially as use increases among young women."
The white pouch blind spot
Tobacco-free nicotine pouches — often called white snus in Sweden — represent a knowledge gap, according to the researchers.
Use of white pouches has grown rapidly in recent years, particularly among young people and women. There are essentially no studies on how these products affect oral health long-term.
Preliminary studies suggest white pouches may cause different oral mucosal changes than those previously observed with tobacco snus, but the data remains thin.
The gap matters: white nicotine pouches are the fastest-growing segment of the oral nicotine market in Europe and North America, yet regulators and public health officials are working with almost no clinical evidence on oral health impacts.
